Supplied video obtained July 30, 2018 of the QLD Coroner's inquest into the infection-related death of an Iranian refugee held in Australian immigration detention on Manus Island. The coroner found his death was "entirely preventable" due to the "compounding effect of multiple errors." The infection developed into sepsis which led to heart attacks and organ failure before Kehazaei was finally evacuated to a Brisbane hospital and declared brain dead. Queensland state coroner Terry Ryan said Monday that "a series of clinical errors, compounded by failures in communication" had led to significant delays in Kehazaei being transferred from Manus Island to PNG's capital Port Moresby and eventually to Australia. Australia's offshore detention regime was not within the scope of the inquest but Mr Ryan said it would be possible to prevent further deaths by relocating asylum seekers to other places, such as Australia. However, he said that was unlikely without a radical revision of policies to deal with asylum seekers who try to get to Australia by boat.
